The Substance Abuse & Mental Health Services Administration provides a substance abuse treatment facility locator throughout the United States using your Zip Code.  The list of facilities covers both substance abuse and other mental health treatments and is very comprehensive.

Gamblers Anonymous provides resources for those of you trying to help a loved one seek help as well as for the gambler himself. 

Addiction, while not a specific center for treatment helps to provide the most up-to-date information on various drugs and addictions while also helping to connect those in need with the proper treatment.

Learn Psychology: Suicide and Depression Awareness for Students aims to provide information and answers for people fighting addiction. They provide extensive information on the multitude of drugs out there as well as the different kinds of treatment options that are available. At the mission is to equip patients and families with the best information, resources and tools to overcome addiction and lead a lifelong recovery.
